Family Consent Agreement

Please fill out one form per household.

Once this form has been submitted, please complete the Learner Information Questionnaire, Registration Form, and submit payment to finalize your enrollment.

I am aware a $400 deposit per household is required at time of enrollment in addition to monthly tuition.

In order for a family to cancel the tuition contract, FLC requires the following:

In cases of economic change/hardship

Two meetings to discuss scholarship opportunities, possible tuition plans, and/or other options for continuing enrollment.

If no agreement can be found by both FLC and the family, tuition and enrollment may be canceled.

In cases of conflict between the family and/or learner and FLC

Three meetings to discuss possibilities for reconciliation, adjustment to program goals, supervision, etc.

If no agreement can be found by both FLC and the family, tuition and enrollment may be canceled.

I give the FLC staff permission to take my learner out on the trails fully supervised for a portion of the day, everyday. While my learner is outside, I understand there are hazards of outdoor activity to include, but are not limited to: injuries from physical activities; injuries from failing to properly use tools such as carving knives and/or fire; exposure to weather and water; exposure to wildlife such as the local amphibians and reptiles; contracting poison ivy; the presence of ticks and of exposure to tick-borne illnesses. I know FLC staff will be removing embedded ticks if they are found.

In an emergency, when the undersigned or other named person cannot be contacted, I hereby authorize the FLC staff to take any action deemed necessary for the best interest of my learner.

FLC staff will exercise many cautions to prevent mishaps (including adequate adult supervision, extreme care in potentially dangerous situations, clear communication with learners), but injuries are still possible. Provided adequate precautions have been taken by FLC staff, I will assume all risks of injury, hereby releasing and holding FLC harmless along with their employees from liability for any such injury.

I represent that I am a legal, responsible adult. I represent and warrant that I am signing this document for myself and my children to participate in the Forest Learning Collective’s nature immersion program for the 2025-26 Year. I am fully aware of the risks and dangers of such participation. I understand that our participation is voluntary, at my and my children’s own risk, and that FLC is not responsible in any manner for my or my children’s personal injury or property damage to me or my children as a result of the usage of any such program. I further understand that I am solely responsible for any personal injury or property damage caused by me or my children as a result of the usage of this program and for other good and valuable consideration, receipt of which is hereby acknowledged, on behalf of myself, my children and all others who may claim by, under, or through myself I do hereby agree to indemnify and hold harmless and do hereby release, acquit, and forever discharge FLC and all of its officers, employees, agents and assigns, and all other persons or companies from any and all claims, actions, or causes of action which I or my children now have, or which may hereafter accrue, whether for personal injury or property damage, whether known or unknown, arising out of or in any way resulting from my and/or my children’s participation in this program. I understand and agree that my signature below represents a signature on behalf of myself and each of my children.

We recognize that the Forest Learning Collective thrives on the ancestral land of the Haudenosaunee Confederacy. This land was and continues to be of great importance to the Haudenosaunee people. Consistent with our values of community and inclusion, we have a responsibility to acknowledge, honor and make visible the collective's relationship to Native peoples.
